This investigation will be conducted by an outside service selected by Schwob Building Company to determine your insurability to drive a motor vehicle owned or leased by Schwob Building Company. At the discretion of company management, automobile insurability may be a prerequisite for hiring. Your driving record will be investigated only with your approval. Your signature indicates that you have read this document, understand it’ contents and have given permission for Schwob Building Company to conduct an investigation of your driving record.
In the event that I become an employee of Schwob Building Company, this form will further authorize Schwob Building Company to periodically check my Motor Vehicle record without any additional signed forms. I understand this authorization is to be part of my permanent employment materials file.

I hereby direct you to release such information upon request of the bearer. I understand that the information release is for official use only by Schwob Building Company, Ltd and may be disclosed to such third parties as necessary in the fulfillment of official responsibilities.
I hereby release any individual, Schwob Building Company Ltd and employees, including record custodians, from any and all liability for damages of whatever kind or nature which may at any time result to me on account of compliance, or any attempts to comply with this authorization.
